The Safe Harbor Transition to Independent Living (TIL) program helps young adults as they transition to adulthood and independent living through supportive case management, life skills development and subsidized apartments.
Safe Harbor serves young adults aged 18-22, providing them subsidized housing in apartments that are located throughout the Merrimack Valley. Our case managers work with young adults on goals related to education, transportation, vocational services, health services, management of finances, access to community social and recreational services, and housing services. Safe Harbor case managers utilize the evidence and trauma-informed Attachment, Self-Regulation, and Competency (ARC) framework in supporting young adults diagnosed with serious mental, emotional or behavioral disorders, complex trauma, and significant mental and behavioral health challenges. ARC helps young adults understand and manage their conditions and behaviors by assisting them to stabilize internal stress and develop competencies to navigate young adulthood. Each young adult works with their case manager to secure affordable housing to successfully transition to living independently in their identified home community upon discharge from the program. We also make it a priority to help all young adults foster healthy relationships with friends, family, and community that lead to a network of lifelong supports and permanent connections.
