NFI’s Foundations for Families (FFF) program provides home-based treatment, community-based outreach, comprehensive assessment, counseling, case coordination and crisis intervention services for youth and families. 

FFF prides itself on being available to families when they need us, with services available on a scheduled basis and in response to a to an immediate crisis. Treatment approaches are trauma- and evidenced-informed and are highly individualized to meet a family’s unique and dynamic needs. Services are directed toward maximizing strengths, enhancing communication, and equipping family members with the supports, resources and skills needed to effectively address conflict and manage dysregulation. The goal of services is to keep families together while avoiding out-of-home placement. The team provides families with information and referrals to community resources to further enhance their support network. 

Foundations for Families staff are highly trained and utilize Attachment, Self-Regulation, and Competency (ARC), an evidence- and trauma-informed clinical framework that supports the caregiving system and assists youth in developing the ability to manage internal states and learn developmental competencies. Staff are also experienced in family engagement, child and adolescent development, serious emotional/behavioral/mental health issues, crisis management and de-escalation skills.  Staffing patterns depend on the service being referred for and may include master’s level clinicians, Case Coordinators, Mentors and/or Parent Partners. 

Foundations for Families services can operate as a “stand alone” service or can be effectively integrated into larger service plans involving other services and providers such as hospitals, state agencies, outpatient services, residential treatment programs and other community-based programs.

Services Provided

In Home Therapy

A team-based approach of providing intense therapeutic services in the home to children/youth with behavioral and mental health needs and their families. 

Therapeutic Mentoring

A mentor works one on one with a child/youth whose behavioral health could benefit from support and coaching to improve daily living, social, communication, behavioral and problem-solving skills.

(Above services available to youth under the age of 21 who are MassHealth Members with insurance through MBHP, TUFTS, OPTUM, WellSense and/or private insurance that provides IHT/TM under BHCA.)

Flexible Support Services

A team-based approach of providing intense therapeutic services in the home to children/youth with behavioral and mental health needs and their families. Teams can be composed of any combination of a clinician, family partner, and/or therapeutic support person.  Services are adapted to the family system, ranging from clinical assessment to support and coaching aimed at improving daily living, social, communication, behavioral and problem-solving skills.

Safe & Stable

Individualized, short-term stabilization and support services for caregivers who have a desire to increase their skills and connect to community resources. Safe and Stable is offered in-home or in the community and assists caregivers in developing and strengthening parenting and co-parenting skills, improving relationships with family, improving techniques to manage youth behavior, and building healthy connections within the community.

Preparation

Short-term stabilization and support for caregivers in preparing themselves, their homes, and their social networks before a child enters or returns to their family. Preparation staff work with caregivers to increase their skills in parenting, communication, and co-regulation.

Youth Support

Individualized, short-term stabilization and support services for youth at risk of out-of-home placement. Offered both in-home and in the community, the program assists youth in developing and strengthening coping skills, improving relationships with family and peers, improving school performance and behavior, and building healthy connections within the community. NFI Youth Support services have three focus areas: Educational/Vocational, Enrichment, and Transition-Aged Youth.

Therapeutic

Services for families and/or youth designed to focus on treating mental and behavioral health challenges and addressing family functioning, caregiver-child attachment, communication, and family conflict. Care is highly individualized and has the ultimate goal of decreasing reliance on professional services while increasing each family’s self-reliance and ability to navigate natural supports and community resources.

Comprehensive

A team-based family stabilization and preservation service with a trauma-informed, solution-focused wraparound approach. Comprehensive services offer treatment planning, counseling, case coordination, family support and skill building, social and recreational services, and crisis intervention services for youth at risk of out-of-home placement and their families. NFI Comprehensive services have three focus areas: Family, Intensive, and Therapeutic.

Substance Use

A 12-week psychoeducational program that focuses on relapse prevention and community supports. For caregivers and/or youth aged 16 to 17, staff offer in-home sessions once to twice per week, along with support for the person served five days per week. NFI Substance Use services include a relapse prevention plan and child safety plan, education and counseling around substance use issues, education about the different levels of community supports, and daily or weekly case coordination.
